Three published cases of bowel ischaemia followed phenylephrine use
A search of cases published from 1995 to 2021 found 18 people whose large bowel lost part of its blood supply, called ischaemic colitis, after taking an oral decongestant: 3 on phenylephrine, 15 on pseudoephedrine. All 18 recovered, one after surgery. The reviewers call it rare but real.
The caveat · Only unusual cases get published
Case reports cannot show that the decongestant caused the colitis. Unusual cases are the ones that get written up and published. With 3 phenylephrine cases, no rate can be worked out.
