59 findings · 17 substances · 27 well-evidenced

Most everyday pains respond to the common painkillers by small margins over placebo, and the ones that do more carry risks to the stomach, kidneys, heart and liver. Supplements seldom beat placebo by an amount a patient would feel.

Physical & bodyAlmost half of acetaminophen liver failures were accidental

Acetaminophen caused 275 of 662 acute liver failures at 22 US centres, and 48% of those were accidents. Of the accidental group, 38% had taken two or more products containing it.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Tertiary referral centres see the worst cases, so 42% is acetaminophen's share of the liver failures that reach those units and never a risk to anyone taking it. Doses in the accidental group were reconstructed from what patients recalled afterwards. The three routes overlap, so one person can sit in all of them and the percentages do not add to a hundred. The narcotic-combination finding is US-specific, since those products are not sold everywhere.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainA 1,000mg dose worked no better than 500mg for pain after surgery

Across 51 trials, about half of people got at least half their pain relieved by one dose, against a fifth on placebo. 500mg and 1,000mg performed the same.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Single doses, in adults, for pain after an operation. That is the setting this drug performs best in, and it is not the setting most people take it in. Adverse-event reporting across the pooled trials was patchy, so the reassuring safety picture is thinner than the efficacy picture. The review dates from 2008 and has not been updated since.Click to keep this openRelieve painAcetaminophen (Paracetamol)
Physical & bodyRaised liver enzymes on the maximum dose did not mean liver damage

At 4 g a day, 31% to 44% had a raised liver enzyme, against none of the 39 people on placebo. Nobody in either trial developed liver disease.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Fourteen days in hospital with every meal provided is not ordinary home use, and 145 people is a small sample for a safety question. The 28-day study used 3 g rather than 4 g, so the two rates are not measuring the same exposure. Its placebo group had an 11% rate of its own, a reminder that this enzyme moves without any drug at all. The 4 g trial ran inside a hydrocodone product development programme, with several industry-affiliated authors.Click to keep this openRelieve painAcetaminophen (Paracetamol)

PainFor chronic back pain, there is no usable paracetamol trial left

A Cochrane search found three placebo-controlled trials in low back pain. The only one covering chronic pain has since been withdrawn, and none was ever found for the subacute form.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.A retracted trial leaves an absence of evidence, not evidence that the drug fails, and only the first of those is true here. Cochrane's inclusion rules were strict, which is why three trials survived a search of a much larger literature. This covers low back pain with no identified cause, not back pain traced to a known injury or disease.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainIbuprofen with paracetamol topped the ranking after surgery

Across 39 reviews and about 50,000 people, the pair at 200 mg and 500 mg gave the best of 53 separately estimated figures: 1.6 people treated for one more to get half their pain relieved.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. No drug was tested against another here. These are numbers from separate trials in separate people, lined up side by side. The authors flag 13 of the drug and dose pairs as possibly affected by publication bias. All of it is a single dose for pain after an operation, which says nothing about taking either drug for weeks. And the arithmetic that makes the pair work is the same arithmetic that makes an unnoticed second acetaminophen product dangerous.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainParacetamol beat placebo by half a point out of 100 for back pain

Across 13 trials on a 0-to-100 pain scale, paracetamol came half a point ahead of placebo, and did not change disability or quality of life. The evidence was graded high quality.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Short-term outcomes only, out to twelve weeks, so this says nothing about months of use. A group average does not rule out individuals who feel better on it. A separate network of 98 trials covering 69 medicines for the same condition rated confidence in every analgesic tested as low or very low, which is a different verdict from a confident null and worth holding alongside it. Four authors, lead and senior among them, had GlaxoSmithKline support for one of the pooled trials, a conflict that runs against the finding rather than towards it.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainFor knee and hip arthritis, the gain was 3.7 points where 9 counts

Paracetamol beat placebo by 3.7 points on a 0-to-100 pain scale. The same research group treats 9 points as the smallest change a patient would notice.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The nine-point line comes from a companion review by the same authors, not from this analysis, and other groups draw it elsewhere. The 192-trial network excluded any trial with fewer than 100 people per group and anything published outside English. Averages hide people, and some individuals in these trials did improve. Patients on paracetamol were also nearly four times as likely to show an abnormal liver function test, a laboratory result the authors say they cannot interpret clinically.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainFor a tension headache, 1,000 mg did nothing at the one-hour mark

Across 23 trials and 8,079 adults, 1,000 mg of paracetamol was no different from placebo at one hour. By two hours, 22 people had to take it for one more to be pain-free.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. This is frequent episodic tension-type headache of moderate or severe intensity, not migraine, which the trials generally excluded, and not the chronic form. Ask an easier question and the number improves: for pain-free or only mild pain at two hours it is 10, and for avoiding a second painkiller it is 7.8. No trial was at low risk of bias on every count, though the reviewers put most of that down to thin reporting rather than bad conduct. PainProducts high in CBD and low in THC may not ease chronic pain at all

Across 25 trials in 2,303 people with chronic pain, what relief was measured, 0.54 and 0.78 points on a ten-point scale, came from balanced or THC-dominant products.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The trials ran one to six months and 64% concerned nerve pain, so this does not cover long-term use or the everyday aches cannabidiol is sold for. PainSeparate CBD from THC and only the epilepsy result survives

Across 152 trials in 12,123 people, cannabidiol's only strongly graded benefit was epilepsy. The pain, spasticity and sleep effects credited to cannabinoids came from THC-containing ones.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Eighty-four separate comparisons across 23 conditions means some significant findings are expected by chance, and most were graded low or very low quality. Physical & bodyThe heart risk is still there at the dose you can buy yourself

Two independent syntheses landed on 1.40 and 1.41 for major cardiovascular events. The community review found the excess still present at over-the-counter doses, at 1.22.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The community data are observational, so doctors steering higher-risk patients towards particular drugs cannot be ruled out. A relative risk says nothing about a given reader's own starting point. In the randomised trials the absolute excess came to about three extra major vascular events per 1,000 people treated for a year, one of them fatal. Those trial doses were high and sustained.Click to keep this openSupport heart and blood flowDiclofenac

Physical & bodyMagnesium did not reduce night-time leg cramps in older adults

Across 11 trials in 735 people, mostly older adults with night cramps, magnesium changed cramp frequency by 0.18 a week against placebo. Nothing here tests cramping during exercise.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The people tested were mostly older adults with night-time leg cramps, not athletes cramping mid-race, so this settles the mineral rather than the sport. The pregnancy trials in the same review could not be pooled at all because their outcomes were reported inconsistently, and a separate Cochrane review graded those same trials as far less biased than this one did. OtherRheumatology's guideline says no to both, with one exception

The American College of Rheumatology recommends strongly against glucosamine and chondroitin for hip and knee. Chondroitin for the hand is its one conditional yes.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. This is a strong recommendation resting on an absence of benefit rather than on demonstrated harm, and the guideline notes glucosamine's toxicity is low. It hardened from a conditional recommendation against glucosamine in 2012, so a reader may still meet the older advice. PainThe pain gain in the largest trials is too small to feel

In trials of more than 200 people each came in 0.3 to 0.5 cm better than placebo on a 10 cm pain scale, short of the 0.9 cm the authors set as noticeable.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The 0.9 cm threshold is these authors' own choice rather than an agreed one, and a reader accepting a smaller threshold would read some of these intervals differently. Restricting by trial size excludes most of the older positive literature, so this is not the body of trials other reviews pool.Click to keep this openRelieve painGlucosamine

PainA prescription drug cleared the bar; the supplements did not

Across 1,583 people over 24 weeks, six in ten on placebo hit the target of a fifth less knee pain. Neither supplement nor the pair beat that. Celecoxib did.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.A placebo response of six in ten leaves very little room for a supplement to show a difference, and every arm could take up to 4,000 mg of paracetamol a day as rescue, narrowing the gaps further. The trial used glucosamine hydrochloride, not the sulfate salt sold in Europe as a medicine.Click to keep this openProtect bones and jointsGlucosamine

PainGiving it two years did not change the answer

662 participants stayed on their assigned capsules for a further 18 months. At two years none of glucosamine, chondroitin, the pair or celecoxib had moved knee pain meaningfully.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. This is a continuation in the participants who met stricter X-ray criteria, not a fresh randomisation, so it carries the parent trial's high placebo response with it. Two years of dropout leaves less power, not more. The authors call the trends beneficial but not significant, which describes a null.Click to keep this openProtect bones and jointsGlucosamine

Physical & bodyThe pair narrowed the joint 0.10 mm less, and pain did not follow

605 Australians took glucosamine, chondroitin, both or placebo for two years. Only the combined arm narrowed less than placebo. Knee pain fell equally in all four groups.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The result is fragile: its interval reaches to 0.002 mm, its p value is 0.046, and it survives only after adjustment for sex, weight and baseline severity. It is one of several comparisons, so a multiple-testing correction would likely remove it. PainThe result for worse knee pain vanished on a second look

Among 354 people starting with moderate-to-severe pain the pair beat placebo, 79% against 54%. A trial enrolling that exact group, 164 people, found no advantage.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The subgroup was one of two specified in advance, but was never powered as a test of its own and no independent trial has repeated it. The Spanish trial is small, at 164 people, and its placebo group's larger improvement appears only when everyone randomised is counted, vanishing among completers.Click to keep this openProtect bones and jointsGlucosamine and Chondroitin (Combination)

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. All five datasets shared were independently funded, so the form distinction is tested only inside independent trials, and no dataset from the manufacturer whose product the argument is about was contributed. ★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. These were high, sustained, prescription-level doses in people with arthritis, not occasional tablets. Every regimen studied raised upper-gut complications, so this is a ranking inside a class that all carried the risk, not a safe option beside a risky one. PainThe gain on long-term back pain was 3.3 points out of 100

In six placebo-controlled trials of non-specific chronic low back pain, disability moved 0.85 points on a 0 to 24 scale. In the better-run trials the pain gap narrowed further still.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Median follow-up was 56 days, far too short to pick up the harms that build with long-term use, and the review says outright that it cannot make firm statements about safety. The reported adverse-event ratio of 1.04 almost certainly understates real-world harm for the same reason. Only English, German and Dutch language trials were eligible. PainFive pills out of everything tested clear what patients can notice

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Diclofenac at 150 mg a day, the most effective oral option in the network, also carried a raised risk of people dropping out because of side effects, so the ranking rewards a preparation more people stopped taking. Only English-language trials with at least 100 patients per group were eligible, which leaves the smaller ones out. This covers knee and hip osteoarthritis, not pain in general. ★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The review rates most of its own evidence as low quality, chiefly because the trials reported their methods poorly, and the studies disagreed with each other substantially on the main pain comparison. Fifty-nine per cent of the trials were commercially funded and a further 31% did not say who paid. ★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. Most of the included trials were at high risk of bias or raised concerns, and all of them ran for a year or less, so nothing here bears on years of combined use. The result for ulcer complications, as opposed to ulcers found by endoscopy, had an interval crossing no effect, from 0.10 to 1.07. In pooled trials after surgery, 2.5 people had to take 400 mg of ibuprofen for one more to get half their pain gone over four to six hours. Diclofenac and naproxen sat at 2.7.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. Most of the underlying data come from wisdom-tooth extraction in otherwise healthy adults, a standardised research model rather than everyday pain. These are single doses measured over four to six hours and say nothing about taking something repeatedly. The overview ran no statistical test between the drugs, so these numbers sit side by side and were never compared. Against a dummy gel, diclofenac in one emulsion needed 1.8 people treated for one more to halve their sprain pain, and ketoprofen gel 2.5. The same actives elsewhere needed over 4.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The best diclofenac figure rests on two studies, and some of the ketoprofen data come from 1980s trials with loosely defined outcomes. Around 5,900 participants' worth of registered but never fully reported efficacy data was identified and could not be obtained, which is the direction publication bias usually runs. ★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.About half the people using the carrier gel alone also counted as a success, roughly twice the usual response to a dummy pill, so part of what people feel comes from the vehicle and the act of rubbing rather than from the drug. Physical & bodyThe antidote works best hours before an overdose feels like anything

Liver injury followed in 6.1% of 2,540 treated patients when the antidote started within 10 hours, and in 26.4% when it started at 10 to 24 hours.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. This was an open treatment series with partly historical comparisons, because withholding an antidote to build a control group was not available. It dates from 1988 and the regimens have changed since, most notably to a shorter two-bag drip. The time bands rest on when people said they took the tablets, which is unreliable after an overdose and missing altogether when doses were spread across days. What survives all of that is the steepness of the gradient, which later guidelines reproduce.Click to keep this openRelieve painAcetaminophen (Paracetamol)

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. One specialist transplant unit between 1992 and 2008, so these are the most severe cases and the mortality figures are not overdose mortality generally. Ingestion times and patterns were reconstructed from records after the fact. ★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Neither number marks a point where injury begins, and reading either as one is the most common mistake made with this drug. This is one product from one manufacturer in one country: other acetaminophen products, Tylenol's own included, print different strengths and different ceilings, and labels outside the United States differ again. ★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.None of these three figures is the dose at which injury begins, and the 10,000 mg line is set deliberately below where harm is expected so that clinicians act early. That threshold is also 200 mg per kilogram of body weight wherever that comes to less, so it is lower for a smaller person. High daily doses of a non-steroidal anti-inflammatory drug carried two to three times the serious upper-gut risk of low ones. Raised heart-attack odds appeared within one to seven days.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. These are relative risks, and the review does not supply an absolute one, which turns heavily on age and ulcer history. What counted as a serious complication varied between the pooled studies. The high-dose randomised data come from prescription-level regimens given to people with arthritis, not from the occasional tablet. Risk did not appear to keep climbing past the first month.Click to keep this openSupport heart and blood flowNSAIDs (Non-Steroidal Anti-Inflammatory Drugs)

Other37% took a second anti-inflammatory in the same week, unknowingly

In a one-week diary of 1,326 ibuprofen users, 37% also took a different non-steroidal anti-inflammatory drug, and 11% went over the labelled daily limit.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.An online self-reported diary, so what people recorded may not be what they swallowed, and volunteers who sign up online are not a random sample. Going over the limit was worked out from the recorded times and doses, never checked against an independent record. The study counted behaviour and not harm, so it cannot say how many of those days caused anyone a problem. It ran in the United States, where pack sizes and single-pill doses differ from other markets.Click to keep this openRelieve painNSAIDs (Non-Steroidal Anti-Inflammatory Drugs)

Physical & bodyOrdinary anti-inflammatories showed no link to a break failing to knit

Across 339,864 fracture episodes, non-selective drugs sat at odds of 1.07 and COX-2 inhibitors at 1.84. Opioids sat at 1.69, with no bone-healing mechanism to explain it.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Opioids showed a similar association, at 1.69, with no proposed bone-healing mechanism at all, which points at more painful or more severe fractures attracting stronger prescriptions rather than at the drugs themselves. Prescriptions filled stand in for drugs actually swallowed, and anything bought over the counter is invisible, which weakens the non-selective comparison specifically. Nonunion was rare, following 0.9% of episodes, so these odds sit on small absolute numbers.Click to keep this openRecover fasterNSAIDs (Non-Steroidal Anti-Inflammatory Drugs)

Physical & bodyIbuprofen sat at 1.84 and piroxicam at 7.43 for serious gut trouble

Against taking nothing, the relative risk of a serious upper-gut complication ran 1.84 on ibuprofen, 3.34 on diclofenac, 4.10 on naproxen and 7.43 on piroxicam.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. These come from observational studies whose definitions of a serious complication and methods of recording who took what varied, which the authors flag themselves. The intervals are wide for the less-studied drugs, running from 1.07 to 17.81 for diflunisal. No absolute risk is supplied, and it turns heavily on age and ulcer history. Physical & bodyOlder people taking one had about 2.51 times the odds of kidney injury

Pooled population studies put the odds of acute kidney injury, a sudden drop in kidney function, at about 1.73 times non-use among adults currently taking one.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. Not one of the pooled studies reported how often acute kidney injury happens without the drug, so there is no baseline here to turn these odds into a personal number, and the authors say so outright. Definitions of kidney injury differed between studies. The estimate for people with existing chronic kidney disease rests on five studies whose own figures ranged from 1.12 to 5.25. PainChondroitin's best evidence: 53 in 100 helped, against 47 on placebo

Cochrane pooled 43 trials in more than 9,000 people and rated almost everything it found poor quality. On its one high-quality outcome the gap was six in a hundred.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThe 10-percentage-point figure usually quoted from it is the one it rates low quality, with high bias risk and heavy disagreement. The benefit became uncertain whenever the analysis narrowed to trials that concealed allocation properly, to large trials, or to independently funded ones.Click to keep this openRelieve painChondroitin Sulfate
PainFor a sudden bad back, the benefit is small and the reviews disagree

Cochrane pooled 32 trials and found these drugs better than placebo for acute low back pain. A network of 98 trials rated every painkiller it tested at low or very low confidence.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceBoth reviews agree any benefit is small, and the Cochrane authors call the size probably not clinically relevant. Low confidence means the evidence cannot settle the question, not that the drugs were shown not to work. Only 7 of the 32 trials in the Cochrane review were judged at low risk of bias, almost half were industry-funded, and most were never registered. Neither review followed anyone long enough to speak to repeated use.Click to keep this openRelieve painNSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
Physical & bodyThe trial that cleared celecoxib did not dose the drugs comparably

A head-to-head trial in 24,081 arthritis patients found celecoxib no worse than ibuprofen or naproxen for heart attack and stroke. Mean daily doses ran 209 mg against 2,045 mg.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceNearly seven in ten participants stopped taking the study drug and more than a quarter stopped follow-up altogether, which blunts every comparison the trial makes. It was funded by celecoxib's manufacturer. A non-inferiority design can show one drug is not clearly worse than another; it cannot show that any of them is safe. Everyone enrolled had arthritis and raised cardiovascular risk.Click to keep this openSupport heart and blood flowNSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
OtherWith two acetaminophen boxes to hand, 46% dosed past the daily limit

Five hundred primary care patients showed how they would take real boxes over a day. Just under a quarter went past 4,000 mg using one product; with two, 46% did.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionPeople were showing what they would do, not observed doing it at home, so this measures intent and understanding rather than behaviour. A public hospital clinic and a community practice were among the four sites, and the sample leans towards lower health literacy, which was itself one of two independent predictors of going over. The threshold scored against is the 4,000 mg liver warning rather than the lower per-product ceiling many labels now print, so against that figure the rates would be higher still.Click to keep this openRelieve painAcetaminophen (Paracetamol)
PainEight small trials sit under the headline soreness result

Thirty-seven effect sizes came from eight trials with 61 participants between them. Pooled, they produced a large reduction in muscle soreness, an effect size of 0.73.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceSoreness is self-reported on a visual scale, and it is a symptom rather than a measure of recovery or performance. Physical & bodySweat testing flagged cramp-prone footballers and nobody else

Among 350 college athletes in eleven sports, sweat rate and sweat saltiness picked out cramp-prone American footballers only, and weakly: 0.65 to 0.72 where a coin toss is 0.5.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionWeak discrimination is not a screening test, and the authors do not offer it as one. Cramp history was self-reported, and sweat was collected during ordinary training, never during a cramp. Forearm patches estimate whole-body sweat sodium indirectly and carry real measurement error. One other significant result among roughly fourteen comparisons, potassium content in women, is what chance produces at that many tests.Click to keep this openRelieve painSodium
PainMatching a prescription drug means less without a placebo arm

606 people took the glucosamine-chondroitin pair or celecoxib for six months and pain halved in both. In this condition placebo groups routinely halve pain too.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThe bar for counting as no worse was 40 points on a 0-to-500 pain scale, which is wide. The pair used glucosamine hydrochloride, the salt an earlier analysis concluded was ineffective, which makes matching a prescription drug harder to square with the rest of this literature rather than easier.Click to keep this openRelieve painGlucosamine and Chondroitin (Combination)
PainOne review has the two doing different jobs, and the pair neither

Pooling 25 trials, chondroitin reduced pain and physical function while glucosamine slowed joint narrowing. On three trials, the two combined did neither.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThis review does not separate trials by manufacturer, by funder, or by how well allocation was concealed, the three things earlier analyses of this literature found decide the answer. Physical & body52% of runners on ibuprofen showed kidney injury, and 34% on placebo

In an 80 km desert ultramarathon among 89 runners, the gap was not statistically significant, and injury here means a creatinine rise measured at the finish.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionEighty-nine runners, and the 18-point gap was not statistically significant, with an interval running from 4 points the other way to 41 points in favour. Kidney injury here means a creatinine rise measured at the finish line, a marker that often settles, not a clinical kidney outcome with follow-up. Runners who lost more weight during the race were independently more likely to be affected, so dehydration and the drug are tangled together in these numbers. Multi-day desert racing in heat is a deliberately stressed setting.Click to keep this openRelieve painIbuprofen
PainPregnancy leg cramps did not respond to magnesium in 332 women

Pooling four trials in 332 pregnant women, oral magnesium neither reduced how often leg cramps struck nor sped up recovery from them, against placebo.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionFour trials and 332 women leave enough room that a small real benefit could still be hiding inside the result. Cramps were defined and counted differently from trial to trial, which blunts any pooled figure. And this is cramps in pregnancy, not the ordinary night-time leg cramps most people mean.Click to keep this openRelieve painMagnesium
PainSame dose, opposite answers on cramps in pregnancy

At 300mg a day, one trial had 86% of pregnant women halve their cramps against 60.5% on placebo; another at the same dose found no gap at all. Eight trials could not be pooled.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThe two trials used different magnesium compounds, so the split may be about which form sat in the capsule rather than about magnesium. Both groups improving by a third in the null trial points at expectation and the natural course of cramps doing much of the work. In the oldest positive trial, blood magnesium never actually rose, which makes it hard to credit magnesium with the improvement. And the trials counted cramps so differently that no single number can be drawn across them.Click to keep this openRelieve painMagnesium
Physical & bodyNaproxen looks heart-safest in trials and riskiest in the records

Pooled randomised trials put it at 0.93 for major vascular events, the only drug group with no excess. Pooled prescribing databases put its first-week odds at 1.53.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceBoth figures are relative, and neither set of authors supplies the baseline that would turn them into a personal number. The randomised trials used high, sustained, prescription-level doses in people with arthritis. The database analysis infers what people took from prescription records, so anything bought over the counter is invisible to it.
