What this means in practice

Say what else you are taking. A man being treated for an enlarged prostate is exactly the man likely to be offered something for erections, and the two prescriptions often come from different clinicians who have not spoken to each other. The adjustment is small, starting at 25 mg once the prostate medicine has settled, and it only happens if someone knows both drugs are in play.

What people say

Almost nothing circulates. The pairing has no popular version at all, which is the unusual thing about it: men take tamsulosin or doxazosin for a prostate that has started waking them at night, and have no idea it belongs in the same conversation as an erection drug.

What the evidence supports

The approved US label requires that a man be stable on his alpha-blocker before sildenafil is started, and that sildenafil then be started at 25 mg rather than the usual 50. That instruction came out of the manufacturer's own study: 100 mg was tried alongside doxazosin in four men, the dose was cut to 25 mg after reviewing what happened to them, and at 25 mg the average extra fall in standing systolic pressure was about 6 mmHg, small enough that the confidence interval crossed zero. Three of the seventeen men still dropped more than 30 mmHg on standing. A review of the interaction pharmacology describes some patients developing orthostatic hypotension on the combination, with the effect appearing less troublesome in men established on long-term alpha-blocker therapy. The class is also not uniform: silodosin, the most selective agent in it, showed no measurable interaction with sildenafil in a randomised study.

How it is supposed to work

Alpha-blockers relax the muscle in blood-vessel walls by blocking the signal that keeps them tightened. Sildenafil relaxes the same walls through a separate chemical route. Neither effect is large on its own, but stacking two different relaxation mechanisms can leave too little tone to hold blood pressure up when a person stands, so the fall shows itself as dizziness or fainting on getting up.

Which way it runs

Sildenafil adds to what the alpha-blocker is already doing, rather than the reverse. Timing matters in one direction: the label requires a man to be settled on his alpha-blocker dose before sildenafil is started, not the other way round.

What is still missing

The averages are reassuring and the individuals are not. A mean 6 mmHg tells you the combination is usually uneventful; three men in seventeen dropping over 30 mmHg tells you who the dose rule is written for.
