What this means in practice

Two things follow. If you take nitrates for angina, in any form, including a spray you only use occasionally, sildenafil is off the table and a doctor should be told rather than worked around. And if you have taken sildenafil, say so before accepting nitroglycerin for chest pain, however awkward that conversation is. The same rule covers poppers, which are chemically the same class of drug even though nobody prescribes them.

What people say

"Don't take Viagra if you're on heart medication." Circulating as a blanket warning about anything cardiac, usually with no named drug, no mechanism and no mention of poppers.

What the evidence supports

In a randomised crossover trial in men taking isosorbide mononitrate for angina, a single 50 mg sildenafil dose dropped standing blood pressure by 52/29 mmHg against 25/15 mmHg on placebo, and with a sublingual nitroglycerin tablet the systolic fall was four times greater than on placebo alone. Against amlodipine, an ordinary blood-pressure drug working through a different pathway, sildenafil added only about 8/7 mmHg, no more than it does on its own. That contrast is the whole point: nitrates are categorically different from other cardiovascular drugs, not merely worse. The approved US label makes nitrates in any form, taken regularly or occasionally, one of only two absolute contraindications, and states that it is not known when a nitrate can safely be given after a sildenafil dose.

How it is supposed to work

Both drugs work on the same chemical signal. Nitrates flood the blood vessels with nitric oxide, which raises a messenger called cyclic GMP that tells vessel walls to relax. Sildenafil blocks the enzyme that clears that messenger away. Give both and the messenger accumulates far beyond what either drug produces alone, so blood pressure does not just fall, it collapses.

Which way it runs

The danger runs both ways and in both orders. Taking a nitrate while sildenafil is still in the body is the same event as taking sildenafil while on a nitrate. The order that catches people out most often is the second one: a man takes sildenafil, gets chest pain during or after sex, and is then handed a nitrate spray by a paramedic or a bystander who does not know what he took.

What is still missing

The blanket warning is too vague to protect the person it is aimed at. It frightens men whose heart medicine is a beta-blocker, where sildenafil is usually fine, while saying nothing about the two situations that actually kill: amyl nitrite poppers taken in the same evening, and the nitrate spray offered for chest pain hours later.
