What the evidence supports
A 2025 Nature Reviews Urology review describes gonadotropins including hCG, alongside aromatase inhibitors and selective oestrogen receptor antagonists, as options that may preserve or restore sperm production in select men receiving testosterone. It reports no randomised evidence that doing so protects fertility outcomes such as conception, and the same review states that testosterone is contraindicated in men trying to conceive. Endocrine Society guidance likewise recommends against starting testosterone in men planning fertility in the near term, without carving out an exception for men taking hCG.
How it is supposed to work
Testosterone taken from outside the body switches off the pituitary signals that tell the testis to work, so the very high testosterone concentration inside the testis collapses and sperm production stalls. hCG mimics one of those pituitary signals, keeping the testis stimulated while blood testosterone is being supplied externally.
Which way it runs
This runs one way. hCG offsets one specific effect of testosterone, the shutdown of sperm production, by supplying the pituitary signal that testosterone taken from outside the body suppresses. Testosterone does not correspondingly antagonise hCG, and hCG does not reduce or reverse the effects testosterone is prescribed for.
What is still missing
The popular version says the fertility problem is solved. What exists is a tactic used in selected patients under specialist care, with no randomised evidence that it protects the outcome men actually care about, which is conceiving a child. Guidance still says a man who wants children soon should not be started on testosterone at all, hCG or no hCG.
