Start taking folic acid as soon as you find out you are pregnant.

The neural tube is at its most vulnerable in the fourth week of development, before most people know they are pregnant, and blood folate needs months of steady intake to reach the level the prevention trials achieved. Around 121 million pregnancies a year worldwide are unintended, so for a large share of people there is no version of that advice that could have worked in time.

The advice is right about the tablet and wrong about the clock. By the time a test turns positive, the window it protects has largely closed, which is why fortifying flour reached people that advice never could.
