Myth check

Nicotinamide riboside is the NAD precursor that actually works, because unlike NMN it is small enough to get into your cells directly. It restores your mitochondria, so your energy, strength and stamina come back.

It does raise NAD, and the dose-response is unusually clean: 100, 300 and 1,000 mg a day raised whole blood NAD by 22, 51 and 142 percent. Beyond that the record is mostly null. In obese insulin-resistant men, 2,000 mg a day for 12 weeks changed nothing measurable about insulin sensitivity, fat mobilisation, liver fat or body composition, and muscle biopsies from the same trial showed no change in mitochondrial respiration, content or shape. In older men it reached muscle and shifted gene expression there, but again left mitochondrial performance unchanged. Where it has done something is inflammation: airway inflammation fell by around half in chronic obstructive pulmonary disease, and circulating inflammatory markers fell in older men. The claim that it beats NMN into cells has not held up either, since a 2026 head-to-head trial found the two comparable and traced both to gut bacterial conversion.

The mitochondrial promise is the one thing that has been tested most directly and failed most consistently, in the same muscle biopsies where the NAD itself clearly arrived. The live question has quietly moved to inflammation, which is not what the product is sold on.

OtherMost of the NAD supplement research has been done in rodents

Of 113 NAD studies from 2010 to October 2025, 80 were in rodents. In the 33 human ones the compounds raised blood metabolites while effects on function were inconsistent.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The search closed in October 2025, so anything published since is missing. The human trials measured such different things that they could not be pooled, which makes the null conclusion a reading of the literature rather than a calculated number. The review also found no outcomes trial at all of intravenous NAD, which is sold commercially.Click to keep this openAge well
Physical & bodyNMN left grip, gait speed and chair-stand time unchanged

Pooled across trials in adults over 60, NMN shifted grip strength by about half a kilogram and gait speed by 0.01 m/s, none of it separable from placebo.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Between two and five trials per outcome, so a real effect could still be hiding in an evidence base this thin. What argues against that is how closely the trials agreed with each other: they scattered almost not at all around the null. The nicotinamide riboside results could not be pooled and ran in both directions, longer six-minute walks in peripheral artery disease against worse physical performance scores in mild cognitive impairment.Click to keep this openBuild strength
OtherThe blood NAD rise is real; what follows from it is not known

Nicotinamide riboside and NMN both raise blood NAD within two weeks and hold it there while dosing continues. The step to anything a person feels is unclosed.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Blood is not tissue. The one trial that biopsied muscle to check delivery found NAD-related metabolites raised there, including the products of clearing the compound away, while the muscle's mitochondria performed no differently. In mice, oral doses of both compounds are taken apart into plain nicotinamide in the liver; the same compounds injected arrive intact.Click to keep this openAge well

OtherFor two years the FDA held NMN outside the supplement rules

NMN notifications stopped for 25 months while the agency held it excluded from the supplement definition, then resumed in November 2025 once that was reversed.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. The notification list records that the FDA responded to each submission on a given date. It does not say whether the response objected, and the agency does not approve dietary ingredients in any case. The submission and response dates come straight from the FDA's own published list; the date of the reversal itself comes from legal and trade reporting, because the agency's letter could not be retrieved.Click to keep this open
OtherThe NAD rise may be your gut bacteria's work, not the pill's

Fourteen days of nicotinamide riboside and of NMN raised NAD about equally, but added straight to blood neither did anything. The authors trace the rise to gut bacteria.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Open-label, fourteen days, healthy adults, so this establishes a route rather than an outcome. The bacterial step was shown in fermentation vessels seeded with human gut microbes rather than watched in the participants themselves. OtherThe researchers running these trials say the case is not made

A review by the principal investigators accepts that NAD falls with age and that precursors raise it safely, then asks for powered trials before any therapeutic claim.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.A narrative appraisal rather than a systematic review, so its selection of evidence cannot be reproduced by anyone else. Its authors include the group running the pharmaceutical NMN trials, which makes this the field appraising itself. Its phrase about raising NAD in blood and some tissues is deliberately narrower than the marketing and should not be read as delivery throughout the body.Click to keep this openAge well
OtherNicotinamide riboside and NMN raised NAD by the same amount

When 65 healthy adults took one or the other for 14 days, circulating NAD rose about equally. Plain nicotinamide, the cheap B3 already in food, did nothing.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Fourteen days and open-label, so the comparison is of absorption, not of benefit, and neither arm measured anything a person would feel. Nine of the twelve authors work for Nestle, whose health science arm sells NAD-related products, which is worth noting mainly because the result cuts against both branded products rather than favouring either.Click to keep this openAge well

OtherSwallowed and injected do not deliver the same molecule

In mice tracked with isotope labels, both compounds injected reached tissues around the body intact. Swallowed, the liver broke them into plain nicotinamide first.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. This was one result among many in a study built to map how fast NAD is made and consumed across tissues, not a comparison it was powered to make. It tracks where the molecule goes, not whether anything follows from its arriving. No equivalent measurement has been made in people.Click to keep this openAge well
OtherHigher doses raised blood NAD, and the trial measured no benefit

At 100, 300 and 1,000 mg a day, whole blood NAD rose 22, 51 and 142 percent within two weeks and held there for eight. The trial measured no benefit at all.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Funded by the company that sells the ingredient, with two of the three authors on its staff and the third the inventor of its licensed patents. Eight weeks, in healthy overweight adults. Every number reported is a blood or safety measurement, so the curve describes how much of the pill turns up, not how much it does.Click to keep this openAge well

ImmuneIn one trial, nicotinamide riboside halved a lung inflammation marker

Forty people with stable chronic obstructive pulmonary disease took it for six weeks. Interleukin-8 in their sputum fell an estimated 52.6 percent against placebo.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. The outcome is a marker in spit, not a symptom: nothing was reported about breathlessness, flare-ups, lung function or quality of life. The confidence interval runs from a 76 percent reduction to a 7.6 percent one, so the size of the effect is barely pinned down. Inflammation in the blood did not change, so the effect did not travel beyond the airway. OtherThe biggest dose tried, 3,000 mg a day, was tolerated for four weeks

Twenty people with Parkinson's disease took it daily for four weeks. Nothing moderate or severe happened, blood NAD rose up to fivefold, and homocysteine ticked up.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Twenty people over four weeks, in a phase one safety trial. Its apparent improvement in Parkinson's symptom scores is not an efficacy finding: the authors traced it to participants on the compound having taken their levodopa more recently before assessment. Four weeks establishes the highest dose tested, not a dose anyone has certified for long-term use.Click to keep this openAge well

Metabolic & weightTwelve weeks at 2,000 mg did not improve insulin sensitivity

Forty obese, insulin-resistant men took 2,000 mg a day for twelve weeks. Insulin sensitivity, glucose handling, liver fat and muscle mitochondria were unchanged.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Men only, all obese and insulin-resistant, so this does not describe healthy or older users, and twelve weeks is not years. The mitochondrial half is a sub-study of the same forty men rather than an independent replication, and the two count once. The trial did not report whether NAD actually rose in these participants, so a failure of delivery cannot be fully separated from a failure of the mechanism.Click to keep this openManage blood sugar

Physical & bodyNicotinamide riboside reached ageing muscle and changed nothing in it

Twelve older men took 1,000 mg a day for 21 days. NAD-related metabolites rose in their thigh muscle and gene expression shifted, but mitochondria did not change.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Twelve men over 21 days is a mechanistic study, not an efficacy trial. What rose was the NAD metabolome, the family of related compounds including clearance products; the paper does not report that muscle NAD itself rose in proportion. The gene-expression changes ran downward in the energy and mitochondrial pathways, the opposite direction to the one the rationale predicts. One author invented the licensed patents behind the product and holds stock in the company selling it.Click to keep this openBuild strength

Physical & bodyNobody has tested whether NAD supplements affect the heart

Both precursors raise 2PY and 4PY, breakdown products that in cardiac patients carried 1.6 to 2.0 times the rate of heart attack and stroke. No trial has joined the two.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThe cohorts were stable cardiac patients, not supplement users, so the absolute risk does not transfer to anyone taking a capsule. The human half is observational, though 4PY at physiological levels switched on the vascular inflammation machinery in mice and a genetic analysis across 106,000 people pointed the same way. Every trial of these supplements that measured the metabolites ran weeks to months, far too short for a heart attack to be an endpoint.Click to keep this openSupport heart and blood flow

Metabolic & weightIn 13 volunteers, fat-free mass rose a little and nothing else did

Thirteen overweight people took 1,000 mg a day for six weeks. Fat-free mass rose 1.34 percentage points; insulin sensitivity, organ fat and inflammation did not.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThirteen people is very small even for a crossover design, which makes the two positive results the fragile half and the long list of nulls the sturdy one. The fat-free mass change was one of many outcomes tested, exactly the setup in which a chance finding appears. Six weeks, in healthy overweight and obese volunteers.Click to keep this openBuild strength

Physical & bodyThe blood-pressure claim traces to a trial that found no effect

Twenty-four healthy middle-aged and older adults took 1,000 mg a day for six weeks. On blood pressure and arterial stiffness the authors reported no effect.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionTwenty-four people in a crossover design, six weeks per arm, all of them healthy, so this is an early tolerability and target-engagement study rather than a test of anything. NAD was measured in blood, which shows the pill is absorbed and metabolised, not that it reached the tissues where ageing is supposed to slow. The trial was investigator-initiated at a university with no declared competing interests, which sets it apart from most of this literature.Click to keep this openSupport heart and blood flow

Taken with something else

What the evidence says about combining Nicotinamide Riboside (NR) with each of these. The marker is how well established the interaction is; a caution is about the hazard, which is a different question.
