Manage stress and anxiety

30 findings · 6 substances · 9 well-evidenced

A few supplements eased rated stress in trials, by amounts nobody can pin down and sometimes matched by placebo. Anxiety as a condition is thinly tested, and several findings concern coming off an antidepressant.

Findings

Mood & wellbeingIn nine cortisol studies, ashwagandha was taken daily for weeks

All nine studies in one cortisol review had people take ashwagandha daily, in studies lasting 30 to 112 days. None tested what one dose does.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The review gives each study's cortisol only at the end of its course, so how soon any fall begins is unknown. The falls it reports are measured against where people started, not against placebo, and one of the nine had no placebo group. The two studies that measured cortisol in saliva disagreed, and one found no change. Extract and dose differ from study to study, from 60 mg to 5 g a day.Click to keep this openManage stress and anxietyAshwagandha
Mood & wellbeingAshwagandha lowered cortisol and stress in highly stressed adults

Across nine trials in 558 people, pooled in one review, cortisol fell against placebo, and so did self-rated stress and anxiety. The people studied were already highly stressed when they started.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The trials recruited adults who already described themselves as highly stressed, so the same fall may not appear in someone whose cortisol starts out ordinary. Four of the nine trials reported mild-to-moderate side effects. Extract and dose differed from trial to trial, so the pooled result describes no one product. The second review's two studies that measured cortisol in saliva rather than blood disagreed, one finding no change at all. In a later 12-week trial of older, overweight adults, stress scores fell about as far on placebo as on ashwagandha, and cortisol was not measured, so the self-rated stress part of this result is less settled than the cortisol part.Click to keep this openManage stress and anxietyAshwagandha
PainSeparate CBD from THC and only the epilepsy result survives

Across 152 trials in 12,123 people, cannabidiol's only strongly graded benefit was epilepsy. The pain, spasticity and sleep effects credited to cannabinoids came from THC-containing ones.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Eighty-four separate comparisons across 23 conditions means some significant findings are expected by chance, and most were graded low or very low quality. The missing cannabidiol effect on pain is an absence of evidence inside these trials, not a tested null.Click to keep this openRelieve painCannabidiol (CBD)

Mood & wellbeingFor most mental health conditions, CBD has barely been tested

Eighty-three studies across depression, anxiety, ADHD, Tourette syndrome, post-traumatic stress and psychosis found scarce evidence of benefit. Very few tested cannabidiol on its own.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The search closed in April 2018, before several cannabidiol-specific trials were published. The one anxiety signal the review did find came from pharmaceutical THC, in people whose main problem was chronic pain or multiple sclerosis, and was graded very low quality.Click to keep this openManage stress and anxietyCannabidiol (CBD)
Mood & wellbeingOrdinary pressure raised the morning surge; burnout went with less

In the pooled research, ordinary work and life pressure went with a bigger cortisol surge in the 30 to 45 minutes after waking. Exhaustion and burnout went with a smaller one.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. Most of the pooled studies took a single snapshot rather than following anyone, so nothing here shows stress moving the surge. The stress measures themselves are a mixed bag, from job strain to burnout, and they do not all point the same way, which is the finding rather than a flaw in it.Click to keep this openManage stress and anxietyCortisol
ImmuneLong stress may leave immune cells less able to hear cortisol

Across 41 studies in mice, monkeys and people, long-running stress left immune cells less responsive to cortisol's anti-inflammatory signal, and inflammation rose anyway.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials. The mechanism is shown far more directly in the animal studies than in the human ones, where nobody has followed it over time. And this is about how cells respond rather than how much hormone is present, so a blood cortisol reading says nothing about whether it is happening.Click to keep this openSupport immunityCortisol
Mood & wellbeingOne 200mg dose of theanine eased stress within an hour, cortisol later

In one trial of 26 adults, a drink carrying 200mg of theanine and three minor ingredients cut rated stress at one hour, not at three; cortisol fell at three hours, not at one.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Thirty-four people finished the trial, but the treatment effects were computed on 26 — the ones who had actually reacted to the task at baseline, picked out after the fact. An industry grant funded the study, and the paper says the funder had no hand in the analysis. The other ingredients were phosphatidylserine, alpha-glycerylphosphorylcholine and chamomile, each in a small dose. Why the two effects arrived two hours apart is unexplained. Performance on the task itself never moved.Click to keep this openManage stress and anxietyL-theanine
Mood & wellbeingA theanine drink may blunt cortisol's rise under stress, in one trial

Three hours after a drink with 200 mg of theanine, a stressful task did not raise the typical person's cortisol; after placebo it did. Resting cortisol was not compared.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Thirty-six people enrolled and 34 finished, but the effects were computed on 26, chosen after testing and before unblinding for having reacted to the task at baseline. Four people gave too little saliva and were left out of the cortisol analysis, and the paper does not say how many remained. The drink was not theanine alone: it also carried 25 mg of alpha-glycerylphosphorylcholine, 1 mg of phosphatidylserine and 10 mg of chamomile. Before dosing and at one hour the task did not reliably raise cortisol, so the rise the drink blunted appeared at one session out of three. A grant from a company, Neurobrands, paid for the trial, and the paper says the funder had no input into its design, conduct or interpretation.Click to keep this openManage stress and anxietyL-theanine
Mood & wellbeingFeeling less stressed on theanine did not mean doing the task better

In one trial of 26 adults, a drink with 200mg of theanine lowered rated stress at one hour and saliva cortisol two hours later. Scores on the stressful task itself did not change.

★★★Well replicated — this finding’s sources average out to meta-analyses, systematic reviews or high-powered trials.Twenty-six of the 34 who finished were analysed, and they were the ones who had reacted to the task at baseline. The drink was not theanine alone: phosphatidylserine, alpha-glycerylphosphorylcholine and chamomile came with it, in small doses. An industry grant funded the trial, and the paper says the funder had no hand in its design or analysis. A brain-wave finding in the same trial turned up only in the more anxious participants and did not track the stress or cortisol results, so it stands unexplained.Click to keep this openManage stress and anxietyL-theanine

OtherThe studied dose is 750mg a day; shoppers take 50.3mg

The prescription medicine is dosed by weight: about 750mg a day for a 75kg adult, up to 1,500mg. US adults buying cannabidiol themselves averaged 50.3mg.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. The prescription figures are set per kilogram of body weight; the milligram equivalents here use the label's own 75kg reference, and many of the patients in these trials are children. The consumer average has a standard deviation of 40.7mg, nearly as wide as the mean itself.Click to keep this openManage stress and anxietyCannabidiol (CBD)

OtherPure CBD left a drug test clean; CBD cannabis did not

Eighteen adults took 100mg of cannabidiol four ways and none went above the cannabis screening cutoff. Three of the same eighteen did after vaporising CBD-dominant cannabis, which contains THC.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Single doses in a residential unit, about six people per formulation, so this is not a test of daily use. OtherThe same 100mg of CBD showed up eight-fold apart by product

Eighteen adults took 100mg of cannabidiol three ways. Peak urine levels reached 1,274 nanograms per millilitre from the prescription solution, 776 from a capsule and 151 from a syrup.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Urine, not blood, so these concentrations cannot be set beside the plasma figures on the prescription label. OtherTwo CBD products in three did not hold what the label claimed

Of 84 products bought online, 30.95% held cannabidiol within a tenth of the claimed amount, and more held too much than too little. A later batch of 80 put that at 54%.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. Each study sets its own tolerance — a tenth either way here, a stricter band for the drinks — so the percentages describe different tests and should not be pooled. The samples are snapshots too: one online channel in 2016, one US state later, with single-figure counts inside some categories.Click to keep this openManage stress and anxietyCannabidiol (CBD)

Other18 of 84 CBD extracts held THC that the label never mentioned

Of 84 extracts sold as cannabidiol, 18 held detectable THC at up to 6.43mg/mL. A survey of 25 hemp oils found three over the legal THC limit.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. Both are dated regional snapshots — products bought online in 2016 and in one US state in 2020 — and neither describes what is on shelves now. The synthetic cannabinoids turned up in vaping products specifically, so nothing here says the same of oils or edibles.Click to keep this openManage stress and anxietyCannabidiol (CBD)

ImmuneThe folk idea that stress makes you ill has a mechanism under it

Chronic stress wears down cortisol's own off-switch and leaves cells less able to answer the hormone. That pattern turns up alongside lupus, rheumatoid arthritis and multiple sclerosis.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. This is a narrative review drawing human and animal work together, not new data. And the arrow could run either way: a stress-disrupted cortisol system might raise autoimmune risk, or years of autoimmune illness might be what disrupts the system. SleepCortisol before bed predicted the night, and the night the next day

Across 15 days in 95 young adults, a higher cortisol reading before bed came before a shorter, less efficient night, and the shortest sleepers had a flatter curve the next day.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. The authors describe the effects as small. Everyone measured was young and healthy, at an average age of 20, so this says nothing about older adults or about anyone with a sleep or stress disorder. And these are the same people watched over time rather than an experiment, so something else in their days could be driving both ends.Click to keep this openSleep betterCortisol

SleepA night or two of short sleep raised evening cortisol by 30 to 45%

In small experiments, one or two curtailed nights delayed the evening fall by at least an hour and flattened the whole day's curve. Nobody reported feeling more stressed.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier. These were tiny experiments, 13 men in one of them, run over one or two nights. All the participants were young men, so nothing here covers women or older adults. Mood & wellbeingDepression and anxiety scores did not move against placebo

Eleven trials and 312 people: depressive, anxiety and obsessive-compulsive symptoms all came out non-significant. The anxiety side rests on 70 people across four trials.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Eleven trials and 312 people in total, and the authors name the small number of studies as their main limitation. The search closed in August 2013 and nothing comparable has replaced it. Mood & wellbeingOf all antidepressants, venlafaxine is among the hardest to come off

It ranked third in a worldwide database of withdrawal reports. Its short half-life is the likely reason: fast-clearing antidepressants draw about five times the withdrawal reports of slow ones.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.A report count is not a rate: it says withdrawal made up a large share of a drug's reports, not how many users had it. The drugs with the strongest signals were also the most reported and most talked about, which the authors say may inflate their ratios. Among 19 people who had once stopped fluoxetine, a drug that lingers for weeks, remembered withdrawal was as severe as after venlafaxine.Click to keep this openSupport moodVenlafaxine

Physical & bodyWithdrawal usually begins within days of stopping and fades over weeks

Symptoms, which can include shock-like electrical sensations, tend to start within days of stopping. Most clear within weeks; for a minority they last months, for some over a year.

★★☆Single study — solid evidence, such as one good trial or a large observational dataset, but not replicated at the top tier.Twenty-three of the 61 reports were single case reports, and no pooled figure exists for how many people get a prolonged course. The label's drawn-out cases come from voluntary reports with no denominator. NICE puts the usual course at one to two weeks, occasionally months.Click to keep this openSupport moodVenlafaxine

Contested here

The evidence disagrees with itself, or the finding carries a caution. Either way the marker shows one star, however strong the individual sources are.

OtherNobody has watched ashwagandha's cortisol drop past 16 weeks

The trials ran 16 weeks at most and reported no serious side effects. Past that nothing has been measured, and the review that gathered them asks for medical supervision past the short term.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThis is a gap in what has been measured, not a recorded harm: no trial reported an adrenal problem, because none of them looked for one. Mood & wellbeingFour weeks of theanine may ease low mood and anxiety and help sleep

In one trial, 30 healthy adults took 200mg of theanine a day for four weeks. Self-rated depression and anxiety fell, parts of a sleep questionnaire improved, and word fluency gained.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. Mood & wellbeingAshwagandha eases stress in 22 pooled trials; by how much is unclear

The broadest pooling found stress, anxiety and depression scores falling on ashwagandha, and more at higher doses. The size it reports is several times what earlier trials found.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceA pooled stress effect of nearly six standard deviations would mean almost nobody on placebo scored as low as anyone on ashwagandha, while earlier trials of the herb measured effects of 0.63 to 0.74 standard deviations. The authors themselves note the trials differed widely, and pooling very different trials can produce a size no single trial supports. The dose-response finding is read off the same pooling.Click to keep this openManage stress and anxietyAshwagandha
Mood & wellbeingAshwagandha did not beat placebo for stress in one 12-week trial

Over 12 weeks in 120 overweight adults aged 40 to 75, stress scores fell about as far on placebo as on ashwagandha. Fatigue fell further on ashwagandha than on placebo.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidencePlacebo groups in earlier ashwagandha stress trials barely moved, falling around 5.5%, so a 36% placebo fall may be what hid the difference here rather than a weaker herb. Cortisol was not measured, so this says nothing about the hormone. The fatigue result was one secondary outcome among many measured, which makes a chance finding more likely. The trial was paid for by the extract's maker.Click to keep this openManage stress and anxietyAshwagandha
Mood & wellbeingThe whole case for CBD and anxiety is eight small studies

Eight studies with 316 people between them pool into a large effect on anxiety. The uncertainty around it stops just short of no effect at all.

A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThe pooled studies mix one-off experimental stress in healthy volunteers with treating diagnosed anxiety disorders — different questions, answered as one. A large pooled effect resting on trials this small is where the smallest studies dominate, and the authors caution against over-reading it.Click to keep this openManage stress and anxietyCannabidiol (CBD)
OtherAt the doses people actually buy, liver tests looked normal

Among 839 adults taking about 50.3mg of cannabidiol a day for at least 30 days, 9.1% had a raised ALT and 4.0% a raised AST, close to population rates of 8.9% and 4.9%.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThere was no control group — the comparison is to historical population figures, not a concurrent placebo arm. Physical & bodyHigher venlafaxine doses brought more withdrawal, in one 2005 dataset

A UK safety review counted withdrawal in 13% of people on 37.5 mg, 11% on 75 mg and 24% on 150 mg, about 92 to 98 people per dose. The US label states the same trend, without numbers.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThe three dose figures are second-hand: a 2022 review copied them from a 2005 UK regulator's report, and the report itself is where the counts live. The middle dose came out lower than the lowest, so the rise rests on the 150 mg group alone. The label gives the trend and no number.Click to keep this openSupport moodVenlafaxine
Mood & wellbeingYears on venlafaxine may make it harder to stop than months

The label says so, and people surveyed after two years' use had far more withdrawal than those under six months. The short trials never ran long enough to test it.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThe survey reached 18% of those invited, all recalling past attempts, which plausibly inflates every figure, and it did not separate venlafaxine from other antidepressants. The tenfold odds ratio ran from about 3 to 38. The label states the trend without a number. The tapering cohort had no comparison group.Click to keep this openSupport moodVenlafaxine
Mood & wellbeingHow often venlafaxine withdrawal happens has never been pinned down

The big reviews give one rate for all antidepressants. The one head-to-head number, 43.5% moderate-or-worse withdrawal at eight weeks against 34.3% on sertraline, was an unplanned analysis of a trial.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.CautionThe percentages were extracted by a later review from the trial's tables; the trial's own summary gives only the direction. Withdrawal was a side outcome of a trial built to compare how well the drugs worked, after eight weeks of use, so it says nothing about longer use. A review of 61 reports found withdrawal most often with venlafaxine but could give no pooled figure.Click to keep this openSupport moodVenlafaxine
Mood & wellbeingWhether a slower taper means less withdrawal is still unsettled

People tapering in tiny daily steps had less withdrawal the slower they went; in a trial where taper length varied over weeks, it made no difference to withdrawal or relapse.

★☆☆Contested — the evidence disagrees with itself, or the finding carries a caution. A caution or conflict always shows one star, however strong the sources are.Conflicting evidenceThe trial's taper lengths were chosen by the clinic, not randomised, and ran weeks. The cohort had no group tapering without the strips, and the 608 who rated symptoms daily were a self-selected minority of 3,956 users, most of whom had failed before.
